Dear Parents,
The WQSB and Public Health have partnered to provide parents with the option to have their child vaccinated through the school. For your convenience, please use one of the links below to book your child for the COVID-19 vaccine. There are two options outlined along with some important information about the Lord Aylmer vaccination dates.
- Lord Aylmer vaccinations will take place off-site on December 17th and December 20th. Although you will be required to select one of the dates and a time when completing the form, please note that we will be organizing the groups based on numbers and bus availability on vaccination days and therefore, your child may be vaccinated at a different time slot or on the alternate day (17th or 20th) than the one you selected.
- Students from Lord Aylmer will be bussed to South Hull to receive the vaccination.
- Students will be supervised by WQSB and CISSSO staff while at South Hull.
- Parents will not be permitted to accompany their child. If your child struggles with needles and you would like to accompany them, you may want to consider booking your appointment through the public Clic Santé link below.
- If you choose to have your child vaccinated through the school, you must complete and sign the electronic consent form via the Clic Santé link below, at least 24 hours before the day of the school vaccination. It is also possible for you to complete a paper consent (PDF link) in the event that you do not have access to the internet. Please contact your child’s teacher should you require a paper copy sent home.
