Used when a child’s father or mother is a Canadian citizen and the father or mother received elementary instruction in English in Canada and that instruction constitutes the major part of the elementary instruction received by that parent in Canada.
Please complete the necessary documents below and return them to the school along with all supporting documents (originals only). Photocopies will not be accepted. Once all forms are complete, please contact Karen Comfort by email (kcomfort@wqsb.qc.ca) or by phone (819-684-4888) to set up an appointment to complete the registration process.

Forms to be completed:
- Form 1 – Long Application (Code I)
- Section 1 – Complete and Sign
- Section 2 – Check off Code I
- Section 3A – Complete your name, date of birth and fill in the table starting with Grade 1 to high school with the names of the school(s) attended.
- Section 3B – This section must be verified and signed for by your former school or school board depending on who has your student records. Find out where your file is by calling the school board first and ask where you can scan the documents to be signed. Only a Vice-Principal or higher can sign this form.
- Section 3C to 5 – Ignore these sections
- Application for Admission Form
- Consent Form
- Health Information Form
- Parent Survey (K4 and K5 students only)
- Daycare Registration Form (if applicable)
- School Tax Form if you own property in Quebec
- Release and Exchange of Information Form if your child has any learning disabilities and has been seen by a professional.
Documents to be included with application:
- Long Form Birth Document for student (includes parents’ names)
- Proof of Citizenship for parent (Birth Document or Passport). Must be the parent whose education is being used for the application.
- Proof of Address (Quebec Driver’s License OR a lease AND a utility bill)
- Child’s last report card (if applicable)
