Application for an authorization to receive instruction in English under Section 85 of The Charter of the French Language (Temporary Stay)
Please complete the necessary forms below in BLUE INK and return with the documents requested by email as one PDF file (no pictures accepted) to Karen Comfort, kcomfort@wqsb.qc.ca. Confirmation of receipt will be sent to you by email, once the documents have been received to complete the registration process. If you are not able to scan the documents please contact Karen Comfort to set up an appointment to come to the school between 7:30am and 3:00pm Monday to Friday to have copies made for you.
Forms to be completed:
- Form 2
- Complete sections 1 and 2 (select the criteria that describes your situation).
- Complete the declaration 2.1 to 6 that is associated to your criteria code
- If you are on a military stay, complete section 6 and have your commanding officer swear and sign in the proper sections. Must include the end date.
- Application for Admission Form
- Consent Form
- Health Information Form
- Parent Survey (K4 and K5 students only)
- Daycare Registration Form (if applicable)
- School Tax Form if you own property in Quebec
- Release and Exchange of Information Form if your child has any learning disabilities and has been seen by a professional.
Documents to be included with application:
- Long Form Birth Document for student (includes parents’ names)
- Proof of Citizenship for parent (Birth Document or Passport). If you are using a parent’s eligibility in section 2, it must be the proof of citizenship for that parent. If you are using a sibling, it must be the proof of citizenship for the same parent used in the original application(s).
- Proof of Address (Quebec Driver’s License OR a lease AND a utility bill)
- Child’s last report card (if applicable)
- If applying for a military stay – include a copy of your Military ID card (both sides).
- If applying for a military stay and you are posted in Ontario you need to include the following additional documents:
- Copy of your MPRR record
- A written letter from your employer stating that even though you work in Ontario, you have offices in the National Capital Region and therefore could at any time in your career be asked to work in Quebec.
